Specialized Services

  • Expertise in providing appropriate, high quality dementia care specializing in those with a label of challenging/aggressive behavior and a history of repeated hospitalization for behavior management.
  • Assistance with All  Activities of Daily Living including toileting, bathing, dressing, brushing teeth, and feeding when necessary
  • Attention to Emotional & Spiritual needs of each individual in addition to physical needs
  • Residents stay in one familiar location through death.

Staffing

  • Staff to Patient ratio of 1 to 3 direct care workers who are Dementia Specialists.
  • activity director and 3 activity aids for 32
  • 1-1 buddy staffing for all new residents for 4-10 days to reduced trauma associated with move,  to  build trust, learn specific resident needs and connect new resident with possible buddy
  • Additional staff support 1-1 staffing, when necessary, during acute illness, for behavior management and  end of life support
  • 1 Registered Nurse, in each home, on site a minimum of 40 hrs per week and on-call evenings and weekends
  • RN monitoring & interaction with residents on a daily basis.
  • Ongoing mandatory monthly staff training & support meetings for all staff members.
  • Mentor Program – Mentor identified on each shift that is responsible to maintain resident centered focus of all staff, do new staff orientation & provide accountability & ongoing staff support
  • Volunteer Program - Special training for volunteers to become Dementia Specialists in order to communicate effectively with persons with progressive dementia.

 

Healthcare Services

  • Comprehensive pre-admission screening and assessment by Staff RN
  • RN in charge, personally, for all steps of admission process and support for family
  • RN’s familiar with each residents needs provide ongoing communication and support for families
  • Weekly RN assessments to manage health status and need based behaviors of each resident.
  • Monthly In-house Physician Rounds.
  • Daily medication management.
  • Registered Nurse accompanies residents to all Dr. Appointments.
  • RN will accompany resident on all acute emergencies requiring Ambulance transport, ER or hospitalization
  • Transportation to Doctor and Psychiatrist appointments
  • Professional Foot Care provided by RN on a regular basis.
  • Provide palliative and compassionate End of Life Care to residents and their families at the end of their journey.
